Medical Records Transfer

Request your child’s medical records to be sent to a new pediatrician.

If you would like to transfer to a pediatrician at Kidcrew, click here.

If you are transferring your child’s care outside of Kidcrew, we’re here to help make the records transfer process straightforward and secure.

Medical records are released only with parent or guardian consent.

What this page is for

Use this page if:

  • You are moving your child’s primary care to a physician outside of Kidcrew.
  • Your new physician has requested your child’s medical records.

How the records transfer works

  • Step 1
    Complete the Medical Records Transfer Request form.
  • Step 2
    Include the name and contact information of your child’s new physician.
  • Step 3
    Once consent and payment are received, records are securely sent directly to the new physician.

Records transfer fee

  • $45 medical records transfer fee.

This fee covers:

  • Secure processing of your request.
  • Preparation and release of your child’s medical record.
  • Direct delivery to the new physician.

Important

Records are sent only to healthcare providers.

Transfers cannot be processed without signed consent.

Processing times may vary depending on record size.

Why is there a $45 medical records transfer fee?

The fee covers the secure preparation, processing, and delivery of your child’s medical record. This includes reviewing the request, ensuring consent is complete, and sending records directly to your child’s new physician in a secure manner.

How long does a records transfer take?

Once we receive a completed transfer request form, signed consent, and payment, medical records are processed and sent within 30 days, in accordance with regulatory requirements.
Many requests are completed sooner, depending on the size and complexity of the record.

Who will the records be sent to?

Medical records are sent directly to your child’s new physician. For privacy and security reasons, records cannot be released directly to families.

What if I don’t yet have a new pediatrician?

You may submit the transfer request once you have selected a new clinician. Medical records can only be sent to a licensed healthcare provider, so we’re unable to process transfers without recipient details.

Can I pick up or receive a copy of my child’s medical records myself?

For privacy and security reasons, medical records are released directly to healthcare providers only. We’re unable to provide copies of full medical records directly to families.
